Registration Options

You have two options for CODI registration this year: Online or via snail mail.

The online registration form will transmit your information via email and requires a MasterCard or VISA.

Alternatively, you may download a .pdf file of the registration form or a Word document version and mail it with your check, money order or MasterCard/VISA information.
